who complete a W-4 that claims excessive allowances or exempt
status. Please keep in mind that this has not been reviewed by a
lawyer, even if it may sound legalistic. Please let

The way you've
completed your W-4 indicates that you MAY have a large tax liability
on April 15 of next year. While
we do not question your decision, we do wish to ensure you understand
the impact of your choice. Please
sign this form (you'll receive a copy) indicating that we have advised
you of the following:
1.
A high potential tax liability
2.
We are required by law to send certain W-4 forms that claim
exempt status or a high number or allowances to the IRS.
